Theme
The theme of this year’s Forum is “Connecting Care” – Big Ideas, Local Solutions.
Goal
Bringing health care partners together to lead and develop a strong, coordinated health system and deliver quality connected care.
This goal will be achieved by:
• growing leadership
• building relationships
• creating integrated models of care
• establishing a vibrant, high performing culture
Abstract format
Following the success of the AGPN National Forum 2009 table top presentations, the same format will be used this year for the submission of abstracts.
In the table top sessions, there will be multiple presenters, each presenter at a round table of nine participants. Presentations will be a total of 10 minutes duration. Five minutes of this time will be used to present, and the remaining five minutes will be used for extension, adaptation and discussion of the presentation. Presenters move tables at the sound of the bell, and repeat their presentation. This gives participants a chance to hear about a number of innovative approaches, to ask questions for follow up and application in their local environment and to test their own ideas.
To be successful, abstracts will be assessed against the following five criteria:
1. reflects the theme of the forum;
2. outlines an innovative approach;
3. demonstrates the ability for broader application with flexibility for local delivery;
4. is based on evidence and demonstrates objective evaluation results;
5. demonstrates sustainability and involves partners (ideally including the community)
Please select one of the session topics submit your abstract to. They are:
• Population health and community development
• Workforce culture, capacity and support
• Health service design delivery and integration
• Strategic leadership
Please note that laptops cannot be used and that there is only one presenter per presentation.
Abstracts are to be no more than 300 words in length.
